It means that being out in the rain for too long made me literally cold and sick. Now my nose is all stuffed up and I feel like I could use a thousand tissues right now.
It's easy to get sick like this once it starts getting colder outside, and so a lot of people 'catch' it easy.
So I caught it. A cold, I mean.
